Hell, or any sort of afterlife is determined on the persons belief. when you die you go to what hell or its equivalent of your faith. However the Hells are accessible through one another, but their paradise equivalents are not. It is also possible to escape a hell, extremely difficult/near impossible, but possible. As for the paradises there really is no escape, not none, as it has happened only a few times, but the main way to leave a paradise is through reincarnation.
